• Chicago Department of Public Health (CDPH) Food Service Sanitation Manager Certificate

•Summer Festival vendor permit

• Illinois Business Registration / EIN

• General liability insurance ($1M)

• Auto/commercial vehicle insurance

• Valid vehicle registration and driver’s license

• DOT inspection / safety certification

• Fire extinguisher on board (Class K for cooking)

• Ventilation/exhaust hood with suppression system

• Health inspection certificate (current)

• Proof of commissary kitchen agreement

• Food handler certifications for all staff

• Handwashing station on board

• Proper food storage and temperature logs

• Allergen awareness compliance
